For most of the last few decades, the dominant message about alcohol was that a glass of red wine a day was good for you. A little heart protection. Resveratrol. The French Paradox. It was in magazines, in nightly news segments, in the mouths of cardiologists.
That message is wrong, or at least dramatically overstated. The shift in the science happened in a surprisingly compressed window, roughly 2016-2018, when a few large analyses re-examined the older "moderate drinking is good for you" literature and found most of it was confounded by a methodological problem called the "sick quitter" effect. If you stopped paying attention to alcohol research before 2018, your mental model is probably out of date.
The anchor paper for the current view is the 2018 Lancet Global Burden of Disease (GBD) alcohol study, an analysis pooling 592 studies across 195 countries. The conclusion was blunt: the level of alcohol consumption that minimized health loss was zero drinks per week. Not one. Not a glass of wine with dinner. Zero.
That finding was controversial when published, and the science has continued to move since. The best current evidence points to a more precise takeaway than "no safe level." The largest bias-corrected meta-analysis to date (Zhao, Naimi, et al., JAMA Network Open, 2023, pooling roughly 4.8 million participants) found that once you properly separate lifelong abstainers from former drinkers, low-volume drinking is not significantly different from abstaining for all-cause mortality. Mendelian-randomization studies (notably Millwood 2019 and Biddinger 2022), which use genetic variants to sidestep the confounding that plagues observational work, likewise find no protective J-curve. Put together: light drinking is essentially neutral (no benefit, no meaningful harm), while heavy drinking clearly shortens life. Today's leading research does not support "moderate drinking is protective." For life expectancy, light drinking reads as roughly neutral; for cancer, risk climbs with dose from low levels, which is why US guidance since January 2026 is simply to consume less.
Drinks per week: the level of alcohol consumption associated with the lowest overall health loss

The short answer
- GBD 2018 put the "safest level" at zero drinks per week; the newer, larger bias-corrected evidence (Zhao/Naimi 2023, ~4.8M) refines this to neutral for light drinking, harmful for heavy

The 28-million-person Lancet analysis
The Global Burden of Disease alcohol study drew on 694 consumption data sources across 195 countries; its risk analyses covered 28 million individuals. It measured alcohol consumption and tracked outcomes across 23 health conditions: cancers, cardiovascular disease, liver disease, injuries, infectious disease, and more.
The headline findings:
- Alcohol was the seventh leading risk factor for death globally
- Among adults aged 15-49, alcohol was the leading risk factor for premature death and disability
- The level of consumption that minimized total health loss was zero drinks per week
The key methodological innovation: earlier studies often looked at one condition at a time (e.g., cardiovascular disease alone) and might find a U-shaped curve (apparent benefit for light drinkers). GBD 2018 looked at total health impact across all causes. When you add up all the effects (cancers, injuries, liver disease, infectious disease susceptibility), the benefit for cardiovascular disease (if any) is more than offset by harms elsewhere.
The "sick quitter" problem: earlier cardiovascular studies often compared light drinkers to non-drinkers and found the non-drinkers had worse outcomes. The conclusion was "light drinking is protective." But many non-drinkers in those studies had quit because of health problems. They weren't healthy non-drinkers — they were sick ex-drinkers. When studies properly separate lifelong abstainers from former drinkers, the apparent benefit of moderate drinking largely disappears. This is exactly what the bias-corrected Zhao/Naimi 2023 meta-analysis (~4.8M participants) confirmed: correct for the sick-quitter artifact and light drinking lands right on top of abstaining, with no significant mortality difference either way.
The 2022 update: a nuance worth knowing
The Institute for Health Metrics and Evaluation (the group behind GBD) released a 2022 update that slightly softened the 2018 "zero is safest" framing:
- For adults 40+: the update put the theoretical minimum-risk intake at roughly half a drink a day on average (with regional estimates running as high as nearly two drinks a day), and its models showed small cardiovascular benefits at low intakes for older adults. Genetic (Mendelian-randomization) studies undercut that benefit as causal, so we don't bank on it
- For adults under 40: the "zero is safest" finding still holds
- The update modeled less harm at low intakes than the 2018 numbers suggested; it did not rehabilitate moderate-or-heavier drinking
This is a reasonable evolution, not a reversal. For most adults, the honest public health message is:
- Under 40: zero is safest
- 40+: 1-3 drinks a week is roughly neutral for life expectancy, not health-promoting, and not risk-free for cancer
- Past ~7-14 drinks/week: measurable life-expectancy loss begins (about 6 months at age 40)
- ~15+ drinks/week: the loss climbs to 1-2 years, and 4-5 years at the heaviest intakes
At Longlevity, we penalize heavy drinking, where the evidence is unambiguous, and we treat light drinking as roughly neutral for life expectancy. We deliberately don't encode "every drink shortens your life" in the mortality estimate, because the evidence for net mortality harm at low levels doesn't hold up. That is a statement about lifespan, not a recommendation to drink: we give no credit for drinking ("none" is the best-scoring answer), and the cancer evidence and current US guidance both point the same way, which is less. What changed in 2025 and 2026
Three US developments since this post was first written all point in one direction.
- December 2024: the National Academies' congressionally mandated review found that, in observational data, moderate drinkers (up to one drink a day for women, two for men) had lower all-cause and cardiovascular mortality than never-drinkers, and about a 10% higher breast-cancer risk. The committee did not establish causation, and the mortality finding carries the same abstainer-bias problem the 2023 meta-analysis corrected for.
- January 2025: the US Surgeon General's advisory named alcohol the third leading preventable cause of cancer, behind tobacco and obesity: roughly 100,000 cancer cases and 20,000 cancer deaths a year in the US across seven cancer types, with breast-cancer risk rising from about one drink a day. It called for a cancer warning on alcohol labels and for the daily limits to be reassessed.
- January 2026: the 2025-2030 Dietary Guidelines for Americans dropped the one-drink and two-drink daily limits entirely and replaced them with a single line: consume less alcohol for better health.
Put together, the guidance now separates two questions this post used to blur. For life expectancy, light drinking (1-3 drinks a week) still reads as roughly neutral. For cancer, risk rises with dose from low levels, so no amount is good for you, and less is better.
The counter-intuitive insight: alcohol is a carcinogen
Most people know alcohol damages the liver. Far fewer know it's a Group 1 carcinogen, classified by the International Agency for Research on Cancer (IARC) alongside tobacco and asbestos.
Alcohol causally increases risk of:
- Breast cancer (even at one drink per day, the dose-response is linear and starts low)
- Esophageal cancer
- Liver cancer
- Oral and pharyngeal cancer
- Colorectal cancer
The mechanism is direct. Ethanol is metabolized to acetaldehyde, which damages DNA. It also affects hormone metabolism (raising estrogen levels, linked to breast cancer risk) and gut microbiome composition.
This matters because cancer risk is rarely discussed in "moderate drinking" narratives. The cardiovascular story dominates public imagination, but the cancer story is the one that matters more for long-term mortality.
About 4% of cancer cases globally (roughly 740,000 cases a year) are directly attributable to alcohol (Rumgay et al., Lancet Oncology, 2021). In the U.S., the share is somewhat higher. For breast cancer specifically, moderate drinking (one drink a day) is associated with about a 9-10% increased risk compared to non-drinkers. These are modest individual risks, but they compound over decades.
Alcohol and sleep
One effect almost every drinker has experienced but most don't track: alcohol disrupts sleep architecture.
Alcohol does help many people fall asleep faster (it's a central nervous system depressant). But as your body metabolizes it during the night, the disruption begins:
- Reduced REM sleep: the stage critical for memory consolidation and emotional regulation
- Fragmented sleep: more nighttime awakenings, though you may not consciously remember them
- Reduced deep/slow-wave sleep: the restorative stage when growth hormone releases
- Earlier final wake time: especially common with evening drinking
Even one to two drinks within three hours of bed measurably reduce sleep quality. Tracking data from wearables consistently shows alcohol nights produce worse HRV, higher resting heart rate, lower deep sleep percentage.
If you're not ready to cut alcohol but want the biggest sleep upgrade: don't drink within 3 hours of bed. You'll fall asleep slightly slower but wake up much more recovered.
How many actual years?
The cleanest life-expectancy numbers come from a different study than GBD: Wood et al. (The Lancet, 2018), which pooled 83 prospective studies covering nearly 600,000 current drinkers. Measured from age 40, against the study's lowest-intake reference group (up to about 7 US drinks a week):
- Roughly 7-14 drinks a week: about 6 months shorter life expectancy
- Roughly 14-25 a week: 1-2 years
- More than ~25 a week: 4-5 years
Below about 7 drinks a week, the study measured no life-expectancy loss at all, consistent with the bias-corrected neutral-for-light finding above.
These are population averages. Individual outcomes vary enormously based on genetics, other habits, and timing of consumption. But the direction of the evidence is clear: past the low-intake range, more alcohol means more risk.
The highest-leverage change
The single most useful exercise if you drink regularly: track your actual consumption for two weeks. Most people underestimate by 30-50%. A home wine pour is often 2-3 standard drinks; a cocktail can be 3+. On measurement, self-reported consumption routinely captures only about half of what's actually poured. Awareness alone usually reduces consumption without any other intervention.
The principle the current research supports, simplified:
- Under 40: the safest level is zero or near-zero.
- 1-3 drinks per week: roughly neutral for life expectancy, not health-promoting, and a small dose-related cancer exposure. Fewer is better; none is safest.
- Past roughly 7-14 drinks per week at any age: net harm compounds. This is the range where cutting down is a real longevity move.
Two behavior shifts that tend to produce the biggest quality-of-life wins without feeling like deprivation: no alcohol within 3 hours of sleep (protects sleep architecture even if totals don't change, see the sleep post for why that matters), and 3+ alcohol-free days a week (gives liver function time to reset and usually reduces weekly totals on its own).
A few situations where the general framing above doesn't apply and the guidance is clearer: pregnancy or trying to conceive: the CDC, ACOG, and AAP all recommend zero alcohol; no threshold has been established as safe. Certain medications (many antidepressants, anti-anxiety drugs, sleep medications, some antibiotics, acetaminophen at higher doses, and others) have meaningful interactions with alcohol. Check with your pharmacist if you're not sure. Heavy daily use (roughly 15+ drinks per week, or daily drinking for extended periods) makes abrupt cessation medically risky: withdrawal can include seizures and should be managed by a doctor, not willpower.
What doesn't work (or works less than people think)
- "A glass of red wine for your heart": the original research supporting this narrative was methodologically flawed (sick quitter confound). The current consensus does not support alcohol as a heart-protective behavior.
- Switching to "lighter" drinks (beer vs. spirits, light beer, low-ABV wine): the harm is from ethanol, not the vehicle. Lower-ABV options help only if they reduce total ethanol consumed.
- Cutting gradually without a target. The research on sustained behavior change supports specific numeric targets (e.g., "4 per week") over vague intentions ("cut back").
- Exercising to "offset" drinking. Physical activity doesn't undo alcohol's direct effects on cancer risk, sleep, or liver health.
- "I only drink on weekends": the body doesn't care when you consumed the ethanol. Weekly total is what matters for long-term risk.
The bottom line
The scientific picture on alcohol has shifted dramatically over the last decade. The old "moderate drinking is good for you" story was based on flawed methodology that's been substantially corrected. Once you correct for it (Zhao/Naimi 2023; Mendelian-randomization cohorts), the honest reading is neutral-for-light, harmful-for-heavy. The current evidence supports:
- Light drinking is roughly neutral for life expectancy: no benefit, and no measurable mortality harm at 1-3 drinks a week
- It is not risk-free: cancer risk climbs with dose from low levels, and US guidance since January 2026 is simply to consume less
- Measurable life-expectancy loss begins past roughly 7-14 drinks a week (~6 months at age 40)
- Heavy drinking compounds the harm: 1-2 years at ~15-25 drinks a week, 4-5 years beyond that
The good news is that alcohol is one of the more responsive risk factors. The liver repairs itself. Sleep quality improves within days of stopping. Cancer risk reductions compound over years. If you drink and want to reduce, the payoff starts almost immediately.

So is any amount of alcohol "safe"?
The honest answer: no. No amount is risk-free, and no amount is good for you. At 1-3 drinks a week the effect on life expectancy is too small to measure, while cancer risk still climbs with each drink, which is why US guidance since January 2026 is simply to consume less. If you drink and value it, the useful frame is a small, known exposure you are choosing, not a health-neutral habit. Zero is the lowest-risk level.
What about the French Paradox / Mediterranean drinking patterns?
The "French Paradox" (French people eat a lot of saturated fat but have low cardiovascular disease) was originally attributed partly to red wine. Subsequent research has largely attributed it to other factors: the overall [Mediterranean diet pattern](/blog/longevity-diet-what-research-shows/), walking, social meal rituals, less processed food. Red wine's role, if any, is modest compared to the whole pattern. The Mediterranean diet is protective with or without the wine.
Does alcohol damage recover if I stop?
Significantly, yes. Liver function typically normalizes within weeks of stopping, assuming no cirrhosis. Sleep architecture improves within days. Cancer risk reductions compound over years. Breast cancer risk, for example, starts declining within months of cessation. Some damage (established cirrhosis, certain cancers already developed) is less reversible, but for most drinkers, stopping produces measurable improvement.
Is non-alcoholic beer / "zero proof" a good substitute?
From a health standpoint, yes, the harms of alcohol come from ethanol, not the ritual or flavor. Good non-alcoholic alternatives exist now (much better than they used to be) and provide the social ritual without the metabolic cost. If alcohol is socially embedded in your life and you want to reduce total consumption, N/A substitutes are a legitimate tool, not a compromise.
